If you shoot a grenade hack and take all its health before it reaches you, it will not explode. This is one of many reasons why you should always carry a firearm after the cataclysm.
If you lack a firearm, RUN! Take cover in a building or anything else escape its sight. If it explodes on you, kiss your life goodbye...
