All Prime Foo mutations improve the relevant attribute, based on its current base value. This does not include bonuses from the prerequisite mutations, or from bionics or drugs, or penalties from pain, etc.
This mutation first halves the difference between your base strength and 16 (if your stat is lower then 16), then adds 2. It currently rounds odd differences down. Here, if your Strength was:

Also, if you remove any of the Prime Foo mutations, the relevant stat will be decreased to the lowest possible value it could have been before getting the mutation. For example, if you had 13 Strength and got Prime Strength, your Strength would be raised to 16. If you removed the mutation afterwards, your Strength would be lowered to 12.
