Categories
Once a mutation is selected, you will begin to mutate towards it. If any mutations you currently have conflict with this mutation, then one of the conflicting mutations is randomly removed. If you have no conflicting mutations, but don't have any of the prerequisites, you will begin to mutate towards a random prerequisite. If you already have a prerequisite mutation and no conflicting mutations, you gain the selected mutation immediately.
Starting traits do not increase respective category strength, unless you gain them later via actual mutation! On the upside, this does mean that you can take Robust Genetics without placing yourself into the rather undesirable slime category.
Basic traits
Starting traits can sometimes be gained (or rarely lost) through mutation; if you gain one via mutation, you can lose it as normal. If you start with one of them, Purifier will attempt to set you back to it should it be changed.
As with all mutations, they can jump out as part of your evolution to a further mutation (if this further mutation is rolled too early and cannot be gained by itself), or as part of mutation category-enforcement.

Mutation-only traits
In addition to a points value, mutations have a visibility value and an ugliness value. Positive visibility means that the mutation is prominent. This will visibly identify the player as a mutant, resulting in discrimination from mutant-haters and trust with mutants/mutant-lovers. Positive ugliness means that the mutation is grotesque. This will result in a negative reaction from NPCs, even those who are themselves mutated, unless the NPC is a mutant-lover.
Special
A little secret to get you hunting. It cannot be gained through random mutation at all. This link spoils it all.
