 This infected roach has been feeding on the undead and started to mutate chaotically. Extra limbs and growths sprout from its thorax. 

This creature is a dog sized insect. It's a competent fighter with moderate hit points. It's as fast as you, quite clumsy, has moderate armor against bashing weapons and high armor against cutting ones (that includes bullets). Its special ability allows it to nothing, every 40 turns.

It has moderate morale and it's aggressive. Being a insect, it's afraid of being hurt, nearby fires, and can be angered by nearby monsters of the same type dying. It can also be angered by nearby monsters of the same type being attacked, when the player looks hurt.

It can see, can hear noises, is able to smell characters, can produce chitin when butchered, can climb terrain and furniture, is poisonous to eat (produces tainted meat when butchered), stumbles around, and nothing else.

Being made of insect flesh, it's vulnerable to nothing in particular.

Produces roach dirt, every 3 days.
When it dies, nothing unusual happens.
